The victor doesn't receive damage, but steals damage from the target. Permanently. Each consecutive duel won grants 2 extra damage. So goes like 2,4,6,8 etc. The extra damage is not stolen however.

The simple solution would be to increase the rewarded damage. I mean, if you're taking the time to build an item that does nothing for you but tank you up with stats, then this would be a suitable balance for the time invested.
The FUN way would to do a Sven-like aghs. Each win with aghs would award the the full extra damage to the winner, and a proportion of the extra damage (let's say 0.25 to 0.5 of the awarded dmg) to those within a certain range of the fight (like 500 to 1000). It would change the game into a Bounty Hunter ult dynamic where teams would be more active to support a kill for the bonus.

Ags idea: abyssal war
When equipped with aghanims legion commander can select allies and enemies in an AoE which are then forced to fight each other.
All enemies in AoE fight with each other. If an ally is involved he gets Press the Attack buff while he remains in the radius. They also receive a static 10% chance to do legion commander's current level of Moment of Courage
Whats more, ally kills will grant them 4/7/10 bonus damage.
Legion's damage will be the same as current scale. Ability duration also remains the same as the current ability.
For the enemies, this debuff is undispellable. Many passives including some ultimates are silenced. If an enemy kills his own ally, this enemy will get shocked for the duration of 2/3/4 seconds after Duel duration ends. The shock happens because of the intense guilt of killing a comrade. Shocked heroes cannot move and use item actives. They can auto attack and use abilities.
This ability synergises with her overwhelming odds. It can be something equivalent to disrupter's Static Storm and a watered down Winter's Curse
