The Lightning Revenant's Shocking Build (1)

1st, Vanguard ? No. Get Mekansm, it's just flat out better for teamfight/pushing. It helps you win a lot of games doesn't mean that it's a good item.
2nd, straight out Refresher Orb after Aghanim's Scepter ? No. You'll get man-handled down by the whole opponent team. Better get some tank/safe item first. Refresher Orb is something more late-game oriented when your Razor is beefed enough then that additional ulti will make you even better. But not good building that early.
3rd, don't get Helm of the Dominator that early before any core items. I would suggest you to build Drum of Endurance, as it's better because of stats and additional MS/AS that it provides helps Razor a lot. Lifesteal for Razor isn't that bad anyway, just don't get it too early.

DO NOT BUILD Vanguard EVER ON RANGED HEROES!!!
Instead I prefer to build Drums on Razor as it's a nice little speed boost for early-mid game + some health.
