So just looking at your last game (which was with Razor) I would swap out Vanguard with a Mekansm as it will give your whole team a good armor and hp regen buff as well as a 250 hp burst heal. Also if you want to better transition into the late game, since Razor tends to fall off after 35 minutes, I would recommend getting an Aghnaim's Scepter instead of a Sange and Yasha to better your pushing power and damage output.
I agree Vanguard on Razor is very bad because the block doesn't work on ranged heroes, and besides Vanguard isn't a very good item in the first place. Either get Mekansm because even from a purely selfish point of view it's better than Vanguard (more HP, bonus stats, and gives regen as well), or just grab a naked Vitality Booster for HP and build it into heart later. Since you were your team's safe lane carry I wouldn't have recommended Aghanim's Scepter because it's an item for solo mid/pushing Razor not for carry Razor - SnY/BKB/heart/butterfly/whatever is fine for carry Razor. Carry/racecar Razor *does* have solid late game unlike some may think but Mek/Aghs Razor doesn't. Seeing the picks it was pretty clear that the game was going to go late (lots of teamfight from the Dire making early push impossible and a Furion pick), so going for late game Razor and not pushing Razor was fine - you just don't push against Tide/Enigma/SK anyways.
But honestly the problem in this Razor game wasn't the item build, it was that you were underfarmed as hell! I mean, you are your team's main carry, but you have Vanguard - SnY - BkB is a 50 minute game - you can't carry anything with that kind of farm! Well it's not really your fault, your lane sucked because your Dazzle was a horrible support, and afk jungle Axe is one of the worst things that ever happened to Dota. Dazzle had more last hits than you in 10 minutes meaning that he was stealing all your farm. And he was spamming shadow wave and pushing the lane all the time (pushing the lane is exactly how NOT to play support). When you see a support doing horrible stuff like this you need to tell him to stop. Tell your support to go pull to find his own farm. Also, you shouldn't ever let the enemy creeps come under your tower, instead tank the creeps and walk around until your own creeps arrive - if your tower attacks the enemy creeps it pushes the lane, which is exactly what you should avoid. When you are farming the safe lane the creep equilibrium should always be a few units outside of your tower's range, so that you're close to your tower but it doesn't attack the creeps. Always do what you can to keep this equilibrium. When you play carry you should aim for 50-60 last hits in the first 10 minutes - in this game you had 17 last hits in 10 minutes! If your support is stealing your farm or if your jungler is afk farming while you are losing your lane then you need to tell them so they can do something about it.
For your games with Lion your team had no Mekansm and Lion is a very good Mek hero, you could have made a mek. Blink Dagger is also a very good item on Lion to use your disables in the best way during ganks/teamfights because they have pretty short cast range. I hope the Yasha is just trolling :)
For your Phantom Assassin game, you lost your lane because of a creep-skipping Axe (Axe standing behind your tower and tanking the creeps). Creep-skipping Axe is one of the easiest things to counter in DotA : just call your solo mid to gank as soon as you notice it. As soon as you see Axe behind your tower just go like "hey queen of pain, want a free double kill?" and bam, problem solved, it works every time. Most solo mids can gank at low levels easily, QoP is a typical good example. Don't wait a single second, counter him immediately with a gank, he shouldn't survive more that 20 seconds whenever he's behind your tower. I mean hell, call your whole team to TP to your tower and get him out of there, there's no way 2 heroes with no escape mechanism can stand behind one of your towers without them being free kills.

I'm sure he doesn't 'like' to die a lot. As someone who evidently dies less, perhaps you can advise him how to avoid unnecessary deaths rather than posting thinly veiled disdain.
