The hard lane is the lane where a player goes alone facing the enemy heroes by himself. It's called the hard/suicide lane because getting experience and farm is very difficult, also, there's a chance that you might get killed by the enemy team if they play correctly.
The safe lane is the lane where the carry and supports go. The supports make sure the carry is safe and he can farm properly and also help him get kills on the enemy laners if they can. Also, if the carry is doing well, they can just leave to gank another lanes.

Yeah, when playing Dire (the top team), the safelane is the top lane because its the long lane (the tower is nearer to you), and it is your jungle along your tower, so its harder for the enemy to gank you.
When playing the Radiant (the bottom team), the safelane is bottom, for the same reason.
As Hades said, the hardlane is the opposite lane (bottom lane for Dire, top lane for Radiant), and is usually one guy soloing against the tri-lane of the enemy.
