Yeah, if the courier dies it's your fault for bringing it into dangerous territory. For early-mid game this can be easily solved by upgrading it, with it walking it can easily be picked off by anyone roaming through your jungle. If you're a support, it's good to get in the habit of upgrading the courier the second you have enough gold for it.
1.) A "Kicking" system - We always had have a team mate who is a serial feeder and we know that our game would have been and will be so much better without him.
A kicking system hurts more than it helps;
I came from Left 4 Dead where kick reasons (ulthough unstated) were as silly as "You're bad, I don't like your face, I'm inviting my grandmother who's good at this"
While feeders are deemed uncontributional to the game, the kick feature wouldn't work; games where the kick feature was applied are typically 'drop-in-drop-out' scenarios, whereas Dota is a contious game, starting from when you hit the 'Accept' button to ready a game.
