So I came across this picture the other day and ever since I've been wondering "how do heroes use items?" How do Io, Viper and Slardar wear Boots of Speed? How does Nyx Assassin use a dagon if he doesn't have any fingers? So, any other hero/item interactions you've ever wondered about?

The items most likely have enchantments that grant their effects directly to the user.

Meepo makes sense since each Meepo could carry one item (they all wear the boots). Plus, he has a backpack.

A lot of Heroes have item cosmetics. Nyx has a back-mounted Dagon which he triggers with thoughts, Morphling has E-Blade, I believe Drow has a butterfly bow... in a realistic setting, there is only one of each item, meaning that a lot of Heroes won't get items at all.

Also reporting that Viper for having a Vanguard.
